Start with these checks
The request appears in usage records, but the client does not show normal text.
- AI Code With shows usage while OpenCode is blank, loading, or failed.
- Streaming events are incomplete or out of order
- Keep the usage time and model

Confirm the request completed
Match the usage time, model, and status so the record is not from another client.

Separate service output from UI display
If the service returned or billed the request but OpenCode shows no text, inspect the adapter, stream, and client parser.

Collect a minimal reproduction
Record the model, Provider, adapter, time, and original error. Never include the API Key.
